- The customer needs to download the Clinical Questionnaires and Consent Form.
- The customer will be required to fill out a Clinical Questionnaires, which should include details like clinical history, address, and any required medical authorization.
- Please read the consent form and sign and date it.
- Once the forms are complete, it should be sent back to the company via the provided platform (e.g., email or online submission).
Payment the customer proceeds with the payment. Once the payment is confirmed, the order will be processed and dispatched accordingly.
After receiving the PreciONC Kit, kindly follow the instructions in the manual to collect the sample. Once completed, please send it back to us.
